We are full time Consultant Anaesthetists with an interest in Simulation Education, and as such we are experienced in using and teaching debriefing for learning in educational environments. We often wondered why debriefing is not used more widely in clinical practice, and felt that there would be many benefits to having structured learning conversations in the workplace.
Therefore in June 2014, we started to develop a tool to bring this methodology to the clinical environment in a way that would be widely applicable and simple to use for all.
We used our knowledge of debriefing as well as experience working in complex, dynamic environments to design a tool that was short and easy to understand, but powerful enough to deliver real improvements in patient safety and team performance. At all times our aim has been to foster a culture of inclusivity and empowerment, enabling anyone to speak up when they have ideas to improve patient care.
